1. Go to the Santa Cruz Integrative Medicine Home Page: https://santacruzintegrativemedicine.com/ , click the “Log-In to Portal” button in the top right corner.

2. Log in to your Patient Portal and click on on “Questionnaires” off to the right side of the page
3. Fill out Dr.Rachel’s Transformational Medicine Application and if relevant, the PHQ-9 and GAD-7 questionnaires.

2. Fill in all boxes with an asterisk *
3. Be sure to select “Dr. Rachel Abrams” as your provider

4. Remember to check the boxes for privacy information and patient confidentiality, and complete the CAPTCHA prompt.
(Pay attention to what you put as your username and password, you will need those to log in potentially in the future.)

5. Once you hit “SIGN ME UP!” a confirmation email will be sent your inbox.

6. Open the email you used to sign up and click on the blue text to validate your account.

7. Our office manager Glynis will process your registration and let you know when you can log back in to fill out Dr.Rachel’s Transformational Medicine Application and if relevant, the PHQ-9 and GAD-7 questionnaires. If you haven’t received a response from Glynis within 4 days, you can e-mail her at admin@santacruzintegrativemedicine.com or call the office at 831-465-9088

7. In the Portal, click on on “Questionnaires” off to the right side of the page
8. Please fill out Dr.Rachel’s Transformational Medicine Application and if relevant, the PHQ-9 and GAD-7 questionnaires.
Dr. Rachel will be evaluating applications starting in June and continuing through September 2023. She is trying to create a diverse, supportive community and will get back to everyone as soon as possible. If you haven’t heard from Dr. Rachel, it is because she is still considering your application. She will be considering all scholarship requests beginning in mid-September, 2023.
